Pada kali ini kita akan membahas tentang cara mengisi formulir. Mari kita bahas pembelajaran berikut ini.

A form is a document with spaces (also named fields or placeholders) in which to write or select, for a series of documents with similar contents. 

Formulir adalah dokumen dengan kolom (juga disebut bidang atau tempat penampung) tempat untuk menulis atau memilih, untuk serangkaian dokumen dengan isian serupa.

Beberapa kolom yang harus diisi biasanya meliputi :

1. Name [nomor telepon]
2. Phone number [nomor telepon]
3. Gender [jenis kelamin]
4. Place of Birth [tempat lahir]
5. Date of Birth [tanggal lahir]
6. Address [alamat]
7. House number [nomor rumah]
8. Phone number [nomor telepon]
9. Street [jalan]
10. City [kota]
11. State [negara]
12. Zipcode [kodepos]
13. Age [usia]
14. Hobbies [hobi]
15. dan lain sebagainya

Example :

The following is an application letter written by Heru Wibisono. He wants to be a member of “Dead Poet Society of St. Bernard”. He is an avid writer of poetry as well as a contemplating reader of poems written by famous and infamous poets all over the western world. Read the letter and fill out the application form that follows. Use the information from the letter.
